Before you begin

Note - this form will retain your input as long as your browser stays open and your internet connection is not interrupted. 

Make sure you have all the necessary details and supporting documents. 

This includes: 

  1. A cover letter from the nominator outlining why the nominee is deserving of this recognition
  2. A letter of recommendation that endorses the nominee's qualifications.
  3. A resume, C.V., or biographical sketch (press releases, newspaper articles, statistical information, etc.) documenting the nominee's merits in relation to the selection criteria, and describing additional activities outside their professional life.
  4. Two (or more) letters of support preferably written by someone who knows the nominee well.

Nomination deadline is Friday, April 11, 2025.

Induction categories

a) Athlete

  • Minor sports, high school, or amateur athlete who earned honors representing our community in provincial, national and/or international competitions, or
  • Any athlete from North Cowichan who went on to excel at the collegiate, professional, and/or international level.

b) Team

  • Minor sports, high school, or amateur teams who represented our community in provincial, national and/or international competitions.

c) Coach

  • Minor sports, high school, or amateur teams or individual coaches of any sport whose teams/or individuals went on to participate in provincial, national and/or international competitions.

d) Administrator/Builder

  • Any individual who made significant contribution to local athletics through involvement with a youth sports organization.
Selection methodology

a) Nominations
i) Nominations may originate with an individual, club or association and must be signed by the nominator or club/association representatives.
ii) Any unsuccessful nomination will remain active for reconsideration by the committee for the next two subsequent years. If still unsuccessful the nomination may be resubmitted in its entirety after a one year period of hiatus. Normally, only one resubmission will be considered.

b) Research responsibility
i) All Sports Wall of Fame nomination applications must include the following:

  • A cover letter from the nominator outlining why the nominee is deserving of this recognition
  • A letter of recommendation that endorses the nominee's qualifications.
  • A resume, C.V., or biographical sketch (press releases, newspaper articles, statistical information, etc.) documenting the nominee's merits in relation to the selection criteria, and describing additional activities outside their professional life.
  • Two (or more) letters of support preferably written by someone who knows the nominee well.

ii) Nominators must supply sufficient supportive detail with nominations to allow the committee to make a recommendation. If information is missing, the committee may reach out for more information. 

iii) The committee is responsible for validating all of the information submitted on each nomination. It is not, however, responsible for undertaking the initial research.
